Delivered by Dr. Ravindra Ramadwar, President IAPS, during the Inaugural function of the second virtual and 47th Annual Conference of Indian Association of Pediatric Surgeons 2021.

Respected Chief guest Dr. Bhagwat Karad sir, Honorable Minister of State for Finance Government of India and a senior Pediatric Surgeon from Aurangabad, Respected Guest of Honor My teacher and guide Professor Dr. Mrs. S S Deshmukh, former Professor and Head of Department of Pediatric Surgery, KEM Hospital, former Dean LTMMC and LTMGH, Sion, and ex-Vice Chancellor Mumbai University, distinguished guests, my teachers, seniors, and dear friends, I welcome you all for the second virtual and 47th Annual Conference of Indian Association of Pediatric surgeons.

At the outset, I offer my congratulations as well as sincere gratitude to the organizing team of E-IAPSCON 2021 led by the Organizing Secretary and Secretary of Indian Association of Pediatric Surgeons Dr. Ravi Kanojia, Joint Secretary IAPS Dr. Amar Shah, and the entire young, enthusiastic and motivated members of the executive committee for the excellent organization of this entire conference over 8 days. As the Organizing Chairman of the E-IAPSCON 2021, I was pleased that we were able to organize the first hybrid CME on October 17, 2021 in Mumbai. The CME was inaugurated by Dr. Mrs. SS Deshmukh, Dr. Govind Datar, and Dr. RK Bagdi. This was followed by the felicitation of the recipients of IAPS awards for the year 2020 and 2021. Despite meeting online for frequent EC meetings in the past 2 years, the executive committee of IAPS had its first physical meeting with 15 EC members who attended the CME, and the rest of the EC members joined online.

Last year we conducted a series of Webinars from National and International Speakers. The 46th Annual conference was also conducted online under the leadership of President Dr. Parthapratim Gupta. For the first time, combined online elections of IAPS and three chapters of IAPS, namely, Society of Pediatric Urology, Pediatric Endoscopic Surgeons of India, and Pediatric Oncology chapters were conducted successfully last year.

When I took over Presidentship, many activities were planned, and goals were setup. I am happy that despite the ongoing COVID pandemic and subsequent lockdowns, our IAPS executive committee did a wonderful job [Table 1]. Some of our noteworthy achievements are:{Table 1}

Regular monthly Newsletter of IAPS giving updates on all activities by IAPS membersCompilation of Standard treatment guidelines for pediatric surgical conditions keeping in mind the variations in infrastructure across the country. These were prepared with the help of many members of IAPS. We hope these guidelines will be adopted by the National Medical Council and will provide standardized care for pediatric surgical conditions across the countryCompilation of 150 parent information sheets in 10 languagesCompilation of COVID guidelines for pediatric surgeonsMapping of pediatric surgeons across India to help us identify states and districts where there are no pediatric surgeons [Figure 1]Most importantly, we achieved a breakthrough in getting insurance cover for Neonatal surgical conditions. This was achieved in the private insurance sector with Star Health. Myself, IAPS secretary Dr. Ravi Kanojia, Dr. Amar Shah, Dr. Ramesh Babu, his wife Dr. Madhumathi, Pediatrician and Vice President of Star health and Dr. Prakash, Managing Director of Star Health (who is a surgeon himself), had a long meeting to discuss this issue. I am glad that we were able to convince Star Health to come up with an insurance policy that will cover all congenital anomalies, whether internal or external, from the neonatal age group. The proposed policy is already with IRDA awaiting clearance. Once it is cleared by IRDA, I am sure other insurance companies will follow the suit. This will be the game changer for providing quality services for neonates and infants. I would like to request Honorable Minister Dr. Karad sir to help us in getting insurance for all pediatric surgical conditions from all insurance providersBirthday cards were sent online to every member of IAPS as per IAPS databasePediatric Surgery week was celebrated from November 09, 2020 to November 15, 2020, focusing on the antenatal diagnosis of congenital malformations along with preparation of Posters for display in hospitals, outpatient departments, and private clinics. A table calendar of IAPS with a focus on antenatal diagnosis of surgical conditions was prepared and distributed to the general practitioners, pediatricians, and obstetricians to create awareness in the general populationI along with Dr. Ketan Parikh and Dr. Meera Luthra had a long meeting with NITI Aayog member Dr. Vinod Paul to discuss funding for pediatric surgery procedures under various schemes like Aayushman Bharat, RBSK (Dr. Ravindra Vora has approached Ministry of Health, Government of India, many times), CGHS and various state schemes. The problems of differential procedure charges as compared to adult procedures, delays in payments to hospitals, and private hospitals not paying treating surgeons were discussed. We have suggested that there should be similar packages under all schemes. We also suggested that the difference in packages should be based on whether the services are provided in small hospitals, NABH accredited hospitals, JCI accredited hospitals, as also whether the hospital is in town, small city, large city, or metro as the overheads will be different for every settingA proposal of the formation of Academic Council of IAPS under the leadership of Prof Minu Bajpai has been made subject to approval from the general body of IAPS. The proposed academic council will address issues related to the curriculum, various courses and fellowships in Pediatric Surgery and approach the National Medical Council and National Board of Examinations with many suggestions to popularize Pediatric SurgeryInteractive website of IAPS is still under development which will have patient information leaflets, a directory of Pediatric Surgeons near their home, newsletters, members' area where important documentation, and information is storedUpdated IAPS members directory.{Figure 1}

I am extremely proud of what we have been able to achieve in last two years. The current executive team is fantastic. These youngsters who have done a wonderful job for the past 2 years during the ongoing COVID pandemic. They are the future leaders of IAPS. We must encourage them and nurture their talent. Please give them a big round of applause.

Dear Friends, we all know that every year 2.8 crore babies are born in India. Approximately 4.2 lakh neonates will require surgical intervention every year. The last census has shown that, in our country, there are 52 crore children below the age of 18. Excluding cardiac and neurosurgical diseases, approximately 4.7% that is 2.5 crore children, will require pediatric surgical care every year. Our infrastructure is not ready, and we will need a real boost to provide world-class treatment to all the children across the country. We request Honorable Minister Dr. Karad sir to help us in communicating this vital information to the health minister to emphasize the need for setting up pediatric surgical services.

The way forward for pediatric surgery as I see it is:

Increase the number of departments and MCh/DNB pediatric surgery seatsStart 6-year course to attract trainees after MBBS (The proposal is already submitted to the National Medical council by Prof Minu Bajpai and his team)Convince Director of Medical Education and Research of every state to remove the requirement of compulsory bond for MCh Pediatric surgeryTo attract trainees we should encourage juniors, give them the freedom to choose and develop subspecialtyWork with central and state government to create more jobs for pediatric surgeonsExpand the horizon by including adolescent care for complex congenital malformationsTeaching departments should try to raise the age limit from 12 to 18 yearsRotation of trainees in different departments to expose them to different subspecialties.Encourage good clinical and laboratory researchCreate glamour around our specialty. Identify individuals who can do itDelegate responsibility to young enthusiastic, interested members. They can be junior members in departments or in private practiceCreate an all-inclusive healthy atmosphere in the IAPS.

Every year some ongoing projects will be completed, and new projects will be started, some of which will have to be carried forward by the subsequent President, Secretary, and the Executive Committee. I believe that a continuous progressive path taken by IAPS will go a long way to make it a very impactful organization.

It has been a great honor and privilege to serve as President of the Indian Association of Pediatric Surgeons. I sincerely thank you all for giving me this opportunity. I do hope that during my tenure, I have done justice to the confidence and faith that was bestowed upon me. I would like to thank my IAPS executive committee, senior members of IAPS like Dr. Ravikumar, Dr. DK Gupta, Dr. Minu Bajpai, my close friends Dr. Rasik Shah, Dr. Ketan Parikh, Dr. Kishore Panjwani, Dr. Meera Luthra, Dr. S Ramesh, Dr. Prakash Agrawal, Dr. Raju Saoji, Dr. VVS Chandrasekharam, and many more colleagues across the country to help me in making my year as IAPS President smooth and successful. Your blessings and support always encourage me.

At the end of this conference, the new executive team will be elected under the leadership of President-Elect Prof. Y K Sarin. My best wishes to them for a very productive, fruitful year ahead.
